COURSE OBJECTIVES

The Hernia Essentials (HE) Basics & Practical course is a two days intensive lectures & practical skill workshop training, based on current international guidelines (EHS, APHS, SAGES, IEHS and Hernia Surge guidelines) on hernia management. The objective is to identify, select and train selected surgeons, some of whom may be recommended by National Hernia Chapters of APHS. These surgeons would receive comprehensive training (both didactic and practical) on both approaches (Open and Laparoscopy) for abdominal wall hernias (inguinal, ventral, incisional). These hernias leaders would then serve to propagate and disseminate international guidelines based, best hernia practices within their countries and in the region.

  • DAY 1 : HERNIA ESSENTIALS BASICS

    Inguinal Anatomy
    • Anterior (open) view
    • Posterior (endolaparoscopic) view
    Anatomy of Anterior Abdominal Wall TEP Repair for Inguinal Hernia
    • Patient selection, preoperative considerations, anesthesia, patient position, initial access, port positions, surgical technique, post operative considerations, trouble shooting.
    Open (Lichtenstein) Hernia Repair
    • Preoperative considerations, anaesthesia, patient position, surgical technique, post operative considerations, trouble shooting.
    Open Ventral / Incisional hernia repair
    • Principles of open repair, types of open repair, principles of component separation
    Laparoscopic Ventral / Incisional Hernia Repair
    • Patient selection, preoperative considerations, anesthesia, patient position, initial access, port positions, surgical technique, post operative considerations, trouble shooting.
    Classification of hernia Which hernia repair?
    •  Mesh/ non mesh.
    • Laparoscopy / open.
    • TEP / TAPP

    DAY 2 : HERNIA ESSENTIALS PRACTICALS

    Needle handling, suturing & knotting
    • Suturing (peritoneum and bowel) and knotting
    • Extracorporeal knot – Roeder knot
    • Intracorporeal knot
    • Closing peritoneal tear with Endoloop
    Mesh handling
    • Mesh handling (within package only)
    • Transabdominal sutures on mesh
    • Mesh marking on abdominal wall
    • Central suture on mesh
    • Mesh rolling and insertion through port
    • Mesh laying on floor in abdomen
    • Retrieving transabdominal sutures with needle
    • Fascia closure needle
    • Loop technique
    • Tying loose sutures on surface.
    • Mesh fixation with tackers (distance, counter pressure, within margin)
    Port closure technique
    • Fascia closure needle
    • Loop technique
